The Division Director of Care Management provides strategic and operational leadership to ensure the provision of high quality, safe healthcare and customer service to patients and families in need of Care Management services. The Division Director will partner with hospital leadership, OHSU Healthcare staff, community members and other stakeholders to identify opportunities for operational success.
The Division Director is responsible for the provision of administrative care management services in support of the teaching, research, patient care, outreach and public service missions of the University. Specifically, the Division Director will provide operational development, direction, supervision and administration which includes:
Nurse Case Management
Social Work Transitional Care
Transgender Health Program
Division Director has the primary responsibility for the staff and programs which support patient transitions of care to ensure safe discharges and post-acute care placements. Further, s/he ensures that OHSU meets all regulatory requirements related to patient classification and discharge planning.
The Division Director is a resource in the development of and effective use of:
Population management methodologies
Value-based reimbursement models,
Care coordination strategies
Transdisciplinary coordination of care efforts
Complex case coordination
Effective post-acute care partnerships
S/he is a key stakeholder in advancing OHSU performance regarding length of stay, capacity management and readmission rates. The Division Director works collaboratively with the Clinical Resource Management Committee and the Executive Leadership Team of OHSU Healthcare in the determination and implementation of strategies and tactics for the Care Management Department.
The Division Director manages the Care Management Department on behalf of multiple stakeholders. Accordingly, s/he is accountable to, and takes direction from the Chief Nursing Officer and Executive Leadership Team of OHSU Healthcare.
This position may be filled as a Senior Director, depending upon the applicant pool.

Master’s degree in nursing or related field (e.g. MBA with Healthcare Administration concentration, Public Health)
Five (5) years of work experience in related clinical area of practice AND
Five (5) years recent related management/leadership experience
Requires strong leadership, creativity, high energy, knowledge base in cardiac technology, information management, human resource management, strategic planning, financial analysis, and service line management
Individual must be results oriented, disciplined, focused, have excellent presentation and writing skills, excellent analytical and statistical skills, strong team development skills, and excellent communication skills
Registered Nurse, Licensed Clinical Social Worker in the State of Oregon
Requires prioritization of multiple projects, activities, and assignments
Frequent interruptions in an environment of frequent change and fluctuations
May be exposed to excessive auditory and visual stimulation
For inpatient units: accepts 24/7 accountability and responsibility to include possibility of stepping into patient care or clerical role, as needed
For outpatient or special service-type units: accountability and responsibility for overall program during hours of operation and overall program coordination, including stepping into a clinical or clerical role as needed
May be required to attend out of town conferences according to hospital travel and reimbursement policies
Subject to computer work for several as needed
Must be able to deal with competing needs and priorities of internal and external customers including ensuring safe conditions for patients, families and staff
