WellStar Health System, the largest health system in Georgia and headquartered in the suburbs of Atlanta, has retained Witt/Kieffer to assist in their recruitment efforts for a Vice President of Population Health and Care Coordination.

The Vice President (VP) of Population Health and Care Coordination provides System strategic leadership of population health management, care coordination and transitions of care at WellStar Health System.
Reporting directly to the Executive Vice President and Chief Clinical Integration Officer, this instrumental leader will serve on key strategic leadership councils across the System and be the organizational champion for population health and coordinated care for all patients.
The VP will provide executive leadership for the planning, implementation and monitoring processes and systems to ensure the quality of services and the achievement of patient care objectives through case management and integrated care management services. Efforts will include maximally integrating clinical care and operations for patients in all settings, including the advancement of population health management.
