Director of Care Coordination at WellStar North Fulton

The Director of Care Coordination for WellStar North Fulton, part of WellStar Health System works closely with the Vice President Medical Affairs and Senior Leadership to define the general operational structure and guidelines related to care coordination and case management (including Discharge Planning and Social Services).
This individual works in collaboration with nurses and providers to coordinate optimum services for the patients of WellStar North Fulton. He/she serves as an institutional liaison and industry expert for the Medical Staff, fiscal intermediaries, payors, vendors, and administration
The Director of Care Coordination is responsible for the development and implementation of the department’s budget. The director will assure ongoing performance improvement, attending all required meetings, encouraging educational improvement and assuring employees complete required yearly forums, for reviews and competencies. The director develops, evaluates, initiates, consults and advises the hospital in regard to performance and quality improvement and The Joint Commission or other regulatory agency standards related to case management. The director participates in committees with the hospital medical staff to address and solve issues. The director continually updates processes for monitoring and managing cases under all types of insurance.
Required Minimum Education:
Must hold either a Master’s Degree in Nursing or Baccalaureate degree in nursing (BSN) from accredited nursing schools with a Master’s Degree in related field such as Public Health (MPH), Business Administration (MBA), Health Administration (MHA) (Will consider candidates with ability to obtain Master’s Degree within 1 year of position start-date.)
Required Minimum Certification/Licensure:
Registered Nurse, actively licensed in the state of Georgia. Certified Case Manager (CCM) certification required. Must have a current BLS card from the American Heart Association on the first day of employment in position.
Required Minimum Experience:
Minimum of 7-10 total years of experience as a Registered Nurse; includes a minimum of four (4) years of management in care coordination and/or case management.
Required Minimum Skills:
Should possess excellent verbal and written communication skills and relationship building
Knowledge of the health care environment
Leadership that includes such skills as successfully implementing and managing a change process – ability to use systems thinking- and inspire, coach others to be successful
Model the way for professional practice; professionalism and teamwork/collaboration for self/department/medical staff; and possess necessary business skills to manage human and material resources.
Must be a continuous learner who understands health care financing; strategy and operations for running a business unit(s); and effectively employ data and technology to support work processes and make decisions.
Supports shared governance or shared decision making
Manage the design and delivery of care that in based on evidence and focused on quality and safety.
Knowledge of risk management and various accreditation/regulations/licensing requirements assuring that these are upheld while advancing industry standards through the achievement of goals by effective management and measurement of outcomes.
