Kootenai Health, located in Coeur d’Alene Idaho, is currently accepting applications and nominations for their Executive Director of Care Continuum. The Executive Director, Care Continuum is a new position that will provide leadership in the development, implementation, and oversight of a care coordination model which will support patient-centered care delivery across the Kootenai Health system. This care delivery model will ensure efficient resource utilization, quality outcomes, maximize reimbursement and enhance patient satisfaction. The Executive Director will be a strategic leader that will be responsible for the administrative and operational direction of care management across the continuum; including but not limited to, utilization management, discharge planning/care coordination, acute care and ambulatory clinic social work, medical necessity denials and clinical documentation integrity.
This key leader will have responsibility for all care coordination services within the Kootenai Care Network, ambulatory clinics and post-acute facilities. This will include overseeing the development and implementation of standards, systems, policies, and procedures in alignment with organizational strategic initiatives that are focused on patient, quality and financial outcomes. The Executive Director will lead, and/or partner, on Kootenai Health system, community and region-wide efforts to optimize care coordination across the care continuum. This leader will identify business growth opportunities through care coordination activities and assist in developing strategies to achieve the expansion of existing and new business while ensuring compliance with all state and accreditation reporting requirements.
This position and the services related to the care coordination processes ensure a plan of care for patients in all stages of health. The Executive Director will rely on extensive experience and judgment to plan and accomplish goals. This will include working with managed care, information technology and finance divisions for developing key analytics and metrics for decision making. The Executive Director will report directly to the CNO and will have approximately 50 FTE’s.

The main campus at Kootenai Health is located in Coeur d'Alene, Idaho and includes a 292-bed community-owned hospital. Bachelor’s degree or higher in nursing strongly preferred.
Master’s degree in nursing, healthcare administration, healthcare leadership, or business related field is required.
American Case Management (ACM) and Commission for Case Manager (CCM) certifications.
Five years of progressive nursing management/leadership strongly preferred, as well as case management experience for hospital and ambulatory settings required.
Commercial payor experience preferred.
Extensive understanding of case management practices and knowledge of reimbursement methodologies as well as commercial payer experience preferred.
Ability to effectively engage physicians, payers, management-level staff, employees, and stakeholders in order to build partnerships, achieve strategic initiatives, and attain organizational goals.
Excellent and proven leadership skills incorporating accountability, open communication and team building.
