Job Summary/Purpose: The Manager of Case Management and Care Transition reports to the Director of Case Management. Under general supervision, the Manager for Case Management and Care Transition provides supervision and direction to the assigned Case Management Service Line teams to ensure streamlined patient throughput, appropriate resource utilization, and effective discharge planning/transitions of care per state and federal regulations.
Minimum Training and Education: Master’s degree in Nursing or Bachelor’s degree in Nursing and a Master’s degree in a relevant field and a minimum of 5 years of nursing work experience required; three years of which must have been in a supervisory or management role. Extensive knowledge of inpatient case management within a triad model, hospital reimbursement methodology and utilization management experience, disease management, and data analysis experience. The ability to establish collaborative working relationships with diverse groups, medical staff and other health care disciplines is essential. Program and database development experience a plus.
Required Licensure, Certifications, Registrations: Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state required. Current Basic Life Support (BLS) required, either a certification from an American Heart Association (AHA) BLS for Healthcare Providers (or AHA recognized equivalent) or an American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider. Some positions require certification in Advanced Cardiac Life Support. Certification in Case Management preferred.
